Looks like I gave you the wrong download link.

1. Download the latest Piklist here >
2. Rename the folder piklist before you upload to your site.

Let me know if that works.

You may have to do a hard refresh since it’s a CSS change.

@tommydee– looks like the error is generated at Shortcodes Ultimate, but you can still save the shortcode.

On the Piklist side, we just fixed the issue in our latest beta

You are able to add front end forms with Piklist. There are sample forms built into the Piklist Demos. Use those to start with.
